Congratulations on being nominated for an RDA Australia National Volunteer Award in 2024!

We require your consent before your nomination can be forwarded onto your State Awards Committee and the RDA Australia Awards Panel. The process is easy!

Complete this form and click on submit (if you are under the age of 18 you parent or guardian will need to sign for you).
You will receive an email from us which will have a PDF version of your form attached.
Please forward the email onto your RDA centre contact.
